Peru issues two distinct death documents. The medical "Certificado de Defuncion" is generated in SINADEF, the Ministerio de Salud's death-information system, and since May 2024 carries only the certifying doctor's firma digital plus the cause of death. SINADEF then auto-notifies RENIEC, which produces the legal "Acta de Defuncion" (copia certificada, about S/10.30 online with QR, S/12.00 in person) recording the deceased's full name, DNI, age, and the day, hour, and place of death. Older deaths exist only as handwritten municipal OREC ledger entries. For USCIS filings the RENIEC acta is normally the controlling document, though estates and insurers sometimes need the SINADEF certificate too. The certified English translation must preserve both surnames of the deceased, the DNI, the exact place/date/hour, and — on the SINADEF version — the Spanish cause-of-death phrasing (causa basica, intermedia, directa) rendered faithfully without diagnostic reinterpretation. The translator's certification of accuracy and competence is appended to whichever version is submitted.
