The Colombian Birth Certificate (Registro Civil de Nacimiento)
Colombia's Registro Civil de Nacimiento is issued by the Registraduría Nacional del Estado Civil, by notarías, and by Colombian consulates abroad. Modern copies are a single half-page "copia auténtica" on security paper bearing a serial de registro and the holder's NUIP — ten digits beginning with 1 since Resolution 3571 of 2003; older records carry the alphanumeric 10-character NUIP. A lower "notas marginales" block records any later marriage, divorce, adoption or death. USCIS requires the full certified copy translated verbatim, including every marginal note (or the printed "sin novedad"), the serial, the NUIP, and the "Oficina de Registro" code. Because Colombians carry two surnames — paterno then materno — and mothers keep their maiden names, the translator must render both parents' full names exactly, never anglicizing or dropping the second apellido. Handwritten pre-1990s ledger registros and the newer "registro civil digital" both need certification. Note that USCIS wants the civil-registry birth record, not the hospital "certificado de nacido vivo antecedente" that preceded it.

In Colombia, civil-status records come from the Registraduría Nacional del Estado Civil (National Registry of Civil Status), with notarías (notaries) also authorized to register and issue civil records. Colombia has been a party to the Hague Apostille Convention since 2001, so its civil documents need only an apostille — not embassy or consular legalization — issued by the Ministerio de Relaciones Exteriores (Cancillería), which Colombia now generates as an electronic apostilla.
