Deaths in India are registered under the same Registration of Births and Deaths Act, 1969, the certificate issued on Form 6 by the Registrar of Births and Deaths at the Municipal Corporation, Nagar Panchayat, or Gram Panchayat where the death occurred, ideally within 21 days. The medical cause of death is kept on a separate confidential Form 4/4A and is not printed on the standard certificate. Entries include the deceased's name, date and place of death, age, and usually the father's or spouse's name. For USCIS — a widow(er)'s I-360, removing a deceased household member, or estate matters — the certified English translation must reproduce the registrar's seal, register number, and every regional-language field.
