The international version is the "Çok Dilli Ölüm Kayıt Örneği (Formül C)" under the 1976 Vienna Convention, printed in Turkish, German and French — again no English column, so USCIS requires a certified English translation even of Formül C. Both are barcoded e-Devlet printouts; older entries are handwritten ledger extracts. For immigration matters (revoking a deceased petitioner or beneficiary, widow(er) I-360 self-petitions), USCIS needs the decedent's full name, T.C. Kimlik No, and date and place of death legible; a cause of death may appear and should be translated as written. Preserve Turkish orthography and DD.MM.YYYY dates. The certifier's statement must attest Turkish–English competence, and the decedent's name spelling must match the birth and marriage records.
